What is your cancellation/no show policy & fees?
Clients can cancel or reschedule an appointment anytime: If they provide 24 hours' notice. If you cancel an appointment with less than 24 hours' notice or fail to show up, you will be charged the full session fee ($180 for 60 min individual session) This is NOT covered by your insurance. An active debit/credit card is required to be kept on file. If the card declines, you will be responsible to pay this fee. This means that if an appointment is scheduled for 3:00 pm on a Tuesday, notice must be given by 3:00 pm on Monday at the latest.

Financial Agreement that GLMHS client's sign:
-
Authorize GLMHS to charge your card, which will be kept on file using secure systems, for office charges (copay, coinsurance, etc. This includes late cancellation & no show fees in the event of these situations).
-
I understand that if my credit card does not accept the charge, I will immediately make the payment to the practice (unless a payment plan is arranged)
-
I understand that I may cancel this authorization at any time, but by doing so, I acknowledge that the balance owing will be due & paid in full.

Clients are not able to carry balances between sessions. Your balance will be required to pay in full if you wish to attend the next session.

Payment plan terms: A written contract will be signed by you authorizing a minimum payment every 2-4 weeks (example every 4th Friday). This cannot be postponed (example a financial crisis pops up and you request an extension). If your card declines, you will immediately make the payment to the practice within the month or your entire balance will go to collections.

WHEN THE NO SHOW/LATE CANCEL FEE IS WAIVED:
The only exception to this cancellation policy is in the event of a serious or contagious illness or emergency. I offer one grace every six months. Some examples of emergencies are car accidents, deaths in the family or extreme illness. Work issues do not constitute emergencies (ex. boss asks you to come in, forgot you had to work, can't leave your work in time, etc).

This is at the therapist discretion (2 consecutive cancellations due to contagious illness will require proof of illness). This cancellation policy also applies even if missing the appointment was an unintentional act. In the event of prohibitive weather, we can conduct the session on the phone or video. Payment plans can be arranged to offset cost of late cancellation/no show fee.

FREQUENT CANCELLATIONS OR MISSED APPOINTMENT
Frequent cancellations (4 or more in 6 months or 3 consecutive) and/or missed appointments (no show) will result in the termination of treatment. If you have arranged with your therapist to have recurring appointments, the next recurring appointment will stay in the calendar. Therefore, cancel in the portal or text the office phone if you choose to cancel that appointment to avoid a second No Show/Late cancel charge of $180. Text or email is sufficient to provide a date and time stamp. The recurring appointment will be removed after the second consecutive No Show or cancellation.

Although GLMHS may send you text or email reminders about upcoming appointments, this is done as a courtesy and only if you consent to receive such communications by providing us with your email address and cell number. It remains your sole responsibility to keep track of and timely attend all scheduled therapy appointments, whether or not you receive the text or email reminder. It is your responsibility to inform the office if your phone number or email has changed. After 3 consecutive cancellations or no shows, you will not be able to schedule another appointment and will be referred to another provider.

WAIT TIME/GRACE PERIOD
Your wait time is kept to a minimum. Due to the length of time provided for each appointment, it is critical that you arrive on time for your appointments. If you are more than 15 minutes late to your appointment, we will have no choice but to reschedule your appointment and you will be responsible for the $180 fee of a no show. To avoid paying no show fees, we require at least twenty-four (24) hours’ notice for all cancellations (as described above). Both therapist and client have a grace period of 15 minutes.
